Exposure and Response Prevention (ERP) is a specialized form of cognitive behavioral therapy and one of the most effective treatments for obsessive-compulsive disorder (OCD). OCD often creates a cycle in which intrusive thoughts, images, or doubts lead to anxiety or discomfort. In response, people may perform compulsions, seek reassurance, avoid certain situations, or engage in mental rituals in an effort to feel safer or more certain. Although these responses can provide temporary relief, they can strengthen the OCD cycle over time.

ERP helps clients learn a different way of responding to obsessions and anxiety. Exposure involves gradually and intentionally approaching situations, thoughts, images, or feelings that trigger OCD. Response prevention involves practicing resisting the compulsions, rituals, reassurance seeking, or avoidance that would ordinarily follow.

Through repeated practice, clients have opportunities to learn that they can experience uncomfortable thoughts, feelings, and uncertainty without needing to respond with a compulsion.

Treatment is collaborative and progresses at a manageable pace. Clients work with their therapist to identify OCD triggers, compulsions, avoidance, and other patterns that may be keeping the cycle going. Together, they develop exposure exercises tailored to the client’s particular symptoms and goals.

The goal is not to eliminate every intrusive thought or make certain that feared outcomes will never happen. Instead, clients learn to tolerate uncertainty, respond differently when obsessions occur, and rely less on compulsions to manage distress.

With practice, situations that once felt overwhelming can become easier to approach, urges to perform compulsions can become more manageable, and OCD can have less influence over everyday life.
